I find Dolphin doesn't refresh automatically when another process has changed the directory. For example,
1. go to a directory with an archive (in my case, a .zip file)
2. open the archive from Dolphin (opens in Ark)
3. extract one or more files from the archive. I select a file, then press the extract down-arrow to the right of the word 'Extract'
4. Dolphin does not auto-refresh, now that the directory contents have changed.
I find this annoying too.
