Edit: in the dialog that pops up when you click on your button, asking for root's password, it show the command that it is going to run, is it yours, or the original one. (I just noticed this myself.)
The icon had not been there but I did add it after experiencing the problem. I also changed it out to an alternate icon but that doesn't work either. As far as the command that shows up it is the erroneous command, not the correct/current one and that is what I couldn't figure out how to change. I know the correct script (command) but not how to change the icon's default command. AFIK there isn't a way to edit it. If you know otherwise please enlighten me. Thanks.

The icon is just for the button face. You could any icon for the button you want.
The title of my UA is: Root mode &w/set folders
This is what shows up in the left listbox, when can be selected and set as a toolbar button.
I can then select it and add it to the right listbox. (anything here will show up in the toolbar.)
I can't remember what the icon it was assigned by default, but I changed it to the same icon that the orig command used. (Doesn't have anything to do with the command itself, it's just an icon.)
Therefore, something has reassigned your root toolbar button back to default, and not to your UA.
Try looking for the name you assigned to your UA in the left side list and redo it to the right. But make sure you take out the existing root button by highlighting in the right list and using the arrow button pointing left to take it out of the right list. Then put in your UA.
