[SOLVED] Randomly disappearing icons from panel

Icons are occasionally disappearing from the panel next to Kickoff Application Launcher. Once they are gone, they are gone.
I had added a terminal yesterday, today it is not there.
After a few reboots even the Dolphin icon has disappeared. [I also do not find one under Panel Options > Add widgets that corresponds to the preexisting Dolphin]

This behaviour was seen even under 2009 with KDE4.

How to get the Dolphin icon back?

I have solved the issue of getting Dolphin back on to the panel.
I opened Kickoff Application Launcher, Right clicked on File Manager and selected Add to Panel.
